Time zones and T1D: the concepts
Why crossing time zones matters for insulin, what changes for pumps versus injections, and how to plan the shift with your care team.

Crossing a few time zones scrambles everyone's body a little. With T1D there's an extra wrinkle: parts of your insulin routine are anchored to the clock, and the clock is about to move. Understanding which parts care about time (and which don't) turns a stressful topic into a manageable one.
One thing up front: this guide teaches concepts. It will not compute an adjustment plan for you, because a safe plan depends on your insulin, your doses, your itinerary, and your history, and that's your care team's job. Bring them your travel dates a couple of weeks ahead and leave with a written plan. That's the whole strategy. What follows is the background that makes that conversation easier.
The part that barely cares: mealtime insulin
Good news first. Rapid-acting insulin taken for meals follows the meals themselves. When you eat in the new time zone, you dose for that food, same as at home. Corrections work the same way too. For the mealtime side of T1D, time zones are mostly a non-event.
The part that cares a lot: basal insulin
Basal coverage is anchored to the 24-hour clock, and that's where the wrinkle lives.
On a pump or closed-loop system, your basal pattern follows the pump's clock. Change the clock, and the pattern shifts with it. That makes the mechanics simple, but there are still real questions worth asking your care team: when to change the clock, whether to do it in one jump or in stages for a big shift, and how your system's automation behaves while your body clock and the pump clock disagree.
On injected long-acting insulin, your basal dose happens at a time you choose, and moving that time across zones is the puzzle. Shift it all at once and doses can land too close together (risking stacking) or too far apart (risking a gap). The common approaches involve moving the injection time gradually, or holding it steady relative to home time for short trips. Which approach, and the exact schedule, is precisely the thing to get in writing from your clinician, because getting it wrong in either direction has consequences.
East versus west, in one idea
Flying west stretches your day; flying east compresses it. A longer day can open a gap in basal coverage, and a shorter day can squeeze doses closer together. You don't need to memorize rules about which direction does what. You need to know that direction matters, so that "we're flying to Tokyo" and "we're flying to Hawaii" are different conversations with your care team, not the same one.
Smaller print worth knowing
- A handful of zones or less is often barely worth adjusting for, similar to how your body shrugs off daylight saving time. Short hops and short trips sometimes get the "don't bother" plan. Ask.
- Jet lag itself moves glucose. Disrupted sleep, stress hormones, and odd meal times can nudge insulin sensitivity for a few days. Expect some drift while your body catches up, and judge the trip data kindly.
- Devices have clocks too. Pumps, CGM apps, and phones don't all update time zones the same way (some follow the phone, some don't). Sort out the order you'll update things in before you fly, so your data history doesn't turn into modern art.
- In-flight isn't a time zone problem. Long flights mean sitting still, odd meals, and airline schedules. Many travelers find the flight itself needs more attention than the clock change.
